Clinic Policy Kit is operated by Lakeshore IT Consulting ("we", "us"). By using this site or buying from it, you agree to these terms.

Templates and training, not legal advice

The policy kit, training course, articles, risk quiz and all other materials are general educational resources and starting-point templates. They are not legal advice, do not create an attorney-client relationship, and do not guarantee compliance with HIPAA or any other law. HIPAA compliance depends on your practice's specific circumstances, and state laws may impose additional requirements. Have your finished policies reviewed by a qualified attorney or compliance professional.

License

When you buy the policy kit, your practice (one legal entity, including its locations) may copy, edit and use the templates for its own internal compliance. You may not resell, redistribute or publish the templates.

Training access is for up to the number of staff seats included in your purchase, for the access period shown in your account. Seats are for individual people and may not be shared.

Access period and renewal

Purchases include 12 months of access to training and template updates. We'll email reminders before your access ends. Renewal is optional and is never charged automatically. Certificates already issued stay valid and verifiable after access ends.

Refunds

If you're not satisfied, contact us within 14 days of purchase for a full refund.

Limitation of liability

To the maximum extent permitted by law, our total liability for any claim relating to the materials is limited to the amount you paid for them, and we are not liable for indirect or consequential damages, including penalties or fines.
