This policy explains what information clinicpolicykit.com collects and how we use it. Clinic Policy Kit is operated by Lakeshore IT Consulting ("we", "us"); our contact details are at the end of this page.
What we collect
- Risk quiz: your email address, and optionally your name and practice name, plus your quiz answers and score.
- Purchases: your email, practice name and purchase details. Payments are processed by Stripe; we never see or store your card number.
- Training accounts: names and email addresses of staff a clinic manager invites, and their module progress, quiz scores and certificates.
- Help requests: the contact details and message you submit.
- Technical data: standard server logs (IP address, browser, pages requested) kept for security and troubleshooting.
Please don't send us patient information. Nothing on this site requires PHI, and our forms aren't intended for it.
How we use it
- To show and email your quiz results, and to send a short series of follow-up emails about HIPAA compliance. You can unsubscribe from these at any time using the link in every email.
- To deliver what you bought, provide training access, issue certificates and send account and renewal notices.
- To respond to help requests, which may include introducing you to an IT services partner if you ask for hands-on help.
- To keep the site secure and working.
We don't sell your information.
Certificate verification
Each training certificate has a code. Anyone with the code can confirm the learner's name, practice and completion date on our verification page. That's the point of the certificate.
Service providers
We use Stripe (payments) and an email delivery provider to send email. They process data only to provide those services.
Retention
We keep training records and certificates for at least six years so clinics can rely on them for HIPAA documentation. Quiz leads who never purchase are deleted after 24 months of inactivity. You can ask us to delete your information at any time, subject to records we must keep.
